What to Submit
Email all of the following to Michele Sonnati at msonnati@greenwoodsreferrals.org:
- Completed and signed provider information form
- Completed and signed IPCA agreement
- Completed and signed W9 form
- Copy of your current professional license (front and back)
- Your resume or CV
- Copy of your current malpractice insurance certificate (showing $1,000,000 per episode / $3,000,000 aggregate minimum)
Subject line: “Provider Application – [Your Name]”
W9 Filing tip: If you’re self-employed or a single-member LLC, use your social security number, not an EIN. Make sure the mailing address on the W9 is correct for payment processing.
